1. Why does how you breathe matter?
Almost everything your autonomic nervous system does happens without you. You cannot decide to lower your heart rate, widen a blood vessel or stop producing cortisol.
Breathing is the exception. It runs on its own and it takes instructions, and because it is wired into the same circuitry, changing how you breathe changes things you have no other switch for. That is the whole case for breathwork, and it is a good one. What follows is what happens when people actually measured it.
2. What does the research say?
The clearest trial is from Stanford, published in Cell Reports Medicine in 2023. 108 people were randomised to five minutes a day for a month of one of four things: cyclic sighing (a long inhale, a second short sip of air on top, then a long slow exhale), box breathing, cyclic hyperventilation with breath-holds, or mindfulness meditation as the control. All improved mood and lowered anxiety. Breathwork did more than meditation, and cyclic sighing did most of all, with the largest improvement in daily positive affect and the biggest drop in resting respiratory rate[1].
Five minutes. That is the dose that produced this.
A meta-analysis of 12 randomised controlled trials, published the same month, found breathwork produced a small-to-medium reduction in self-reported stress against control conditions, with similar effects on anxiety and depressive symptoms[2]. Small-to-medium is an honest description of the field: real, repeatable, not transformative.
The physiological half is better established than the psychological half. A systematic review of slow breathing, roughly ten breaths a minute or fewer, found consistent increases in heart rate variability and vagal indices, shifts in EEG toward alpha, and reports of comfort, relaxation and alertness[3].
Train that deliberately and it holds. A meta-analysis of heart rate variability biofeedback, which is slow breathing at your own resonance frequency with a screen showing you the result, found a significant reduction in self-reported stress and anxiety across studies[4].
Blood pressure moves too. A meta-analysis of device- and non-device-guided slow breathing found reductions in systolic and diastolic blood pressure[5]. Modest, and in people who mostly had raised pressure to begin with.
And one trial worth knowing for its size problem. Diaphragmatic breathing over eight weeks improved sustained attention and lowered negative affect and cortisol, in 40 people[6]. Interesting, not settled.
3. How does slow breathing work?
Slow breathing, and particularly a long exhale, increases the traffic on the vagus nerve. You can watch it happen: heart rate rises slightly on the inhale and falls on the exhale, and the size of that swing is one of the things heart rate variability measures[3].
There is also a direct wire. In mice, a small cluster of neurons in the breathing pacemaker projects to the brain region that governs arousal; silence those neurons and the animals stayed calm rather than alert[7]. That is the cleanest demonstration that breathing rhythm talks directly to the state of the brain, and it is in mice. Nobody has shown that circuit in you.
The sigh is the other half. Sighing reinflates collapsed alveoli, and a deliberate double inhale before a long exhale is that reflex, on purpose. It is also the pattern that beat everything else in the Stanford trial[1].
4. Which breathing patterns are tested?
These are the four in the hub's breathing guide, with the same honesty about how well each one is evidenced.
The physiological sigh: 2 in, 6 out, with a second sip at the top
For: bringing yourself down quickly. Two or three of them work within a minute; five minutes a day was the trial dose. Best evidenced of the four, and the one that outperformed meditation in a randomised trial[1].
Coherent breathing: 5.5 in, 5.5 out
For: ten minutes of steadiness, and the pattern to train with. Roughly five and a half breaths a minute is near the resonance frequency where heart rate variability peaks for most people. Well evidenced physiologically, with the psychological effects resting mostly on biofeedback studies[3][4].
The long exhale: 4 in, 8 out
For: before sleep. Any pattern where the exhale is longer than the inhale pushes the same lever. Moderately evidenced as a slow-breathing pattern[3]; the specific ratio is convention rather than a finding.
Box breathing: 4 in, 4 hold, 4 out, 4 hold
For: focus under pressure. Weakest of the four. It was in the Stanford trial and it worked, but it worked less well than sighing[1]. Its reputation comes from where it is used, not from the size of its evidence.
How long. Five minutes a day, done daily, is the dose with a trial behind it[1]. Longer is not better-evidenced, only longer.
When. Sighing whenever you need to come down; the long exhale before bed; coherent breathing at any fixed point you will actually keep.
5. What we honestly do not know
Whether the effect survives proper blinding. You cannot blind somebody to how they are breathing. Every trial here compares an active practice against a control the participant can tell apart, and expectation does real work in outcomes people report about themselves[2].
How long it lasts. The trials run four to eight weeks. Nobody has followed people for years.
Whether it beats meditation in general. One trial found breathwork ahead of mindfulness meditation on mood over a month[1]. Meditation's own evidence base is larger and, in a JAMA review, produced moderate evidence of improvement in anxiety, depression and pain and little evidence of superiority over active alternatives[8]. Treat these as two practices with overlapping effects, not a league table.
Whether HRV going up means anything for you today. It is a real signal and a poor scoreboard: it moves with sleep, alcohol, illness and the position you are sitting in.
6. How do you do it?
- Sit or lie down. Nothing about the posture is magic; it just has to be one you will not fidget out of.
- Breathe through your nose where you can. The mouth is not a failure.
- Let the belly move rather than the shoulders.
- Count, or follow the breathing guide in Kratos Hub, which paces it for you: a circle that grows while you breathe in, holds while you hold, and shrinks while you breathe out. It is on the Mental screen, with the four patterns below built into it.
- Five minutes. Then stop, rather than deciding tomorrow's twenty minutes will make up for it.
If you get light-headed, you are breathing too hard or too fast. Slow breathing should feel like less effort than usual, not more.
7. When should you not do this?
Skip breath-holds and any forceful or rapid pattern entirely if:
- You are pregnant
- You have epilepsy or a seizure disorder
- You have a cardiovascular condition, uncontrolled high blood pressure, or a history of aneurysm
- You have asthma or COPD that is not stable
- You have a panic disorder. Attention on the breath can set off the exact sensations that start an attack
- You are in or near water. Breath-holding before swimming can cause blackout without warning
Slow, gentle breathing with no holds is a different thing from hyperventilation-and-hold practices, and this guide's four patterns are all in the first category. If in doubt, ask your doctor before the second.
